Fees payable to Collab Teaching LLC
Fees shall be calculated per student, teacher, location, and availability. Please refer to your teacher for specific pricing.
No further fees shall be charged for traveling or preparation time of Collab Teaching. Fees may be adjusted from time to time and shall become effective after having given the student/parent seven days written notice.
Fees start at 40.00 per 30 minutes
Payments
Invoices will be emailed and must be paid 48 hours in advance to the tutoring session. If invoice is not paid within 48 hours, the tutoring session will be cancelled. Invoices can be paid by debit, or credit card. Contact your designated Tutor to make arrangements for other payment methods.
Upon signing the agreement, you will receive an invoice for the deposit, which will be equal to the first tutoring session. Your deposit will be due within 48 hours of signing the agreement. Once the payment is complete, your booking will be secured on the calendar.

If you are buying a package, you must pay 45.00 to hold/confirm your booking. The 45.00 will go to your bundle package pricing, which is due 48 hours prior to the session.

If the first session is cancelled 24 hours prior to the session, you will be refunded your deposit.
Fees with Family Empowerment Scholarship (formerly Gardiner) -FES- UA
If you have funds in your Step Up For Students account, Collab Teaching will attempt to direct bill. However, you are responsible for payment if the claim through SUFS is denied for any reason. We will send you an invoice for the current balance on the account within 24 hours, which must be paid in full within 48 hours of the next tutoring session. We are unable to contact or dispute anything on your behalf with SUFS. You will have 48 hours through the SUFS portal to “Approve a Provider Reimbursement.” We only accept the FES-UA scholarship. We do require a $20.00 USD deposit (that we will not bill the scholarship) paid upfront, that will apply to your first session to secure your booking.
Schedule of lessons
Effective from the first date of tutoring for four following weeks.

By agreeing to this contract, you are committing to paying for at least 4 sessions per term. Each term consists of 4 sessions. If services continue after 4 sessions, your term will automatically restart.
Obligations of the student/parent/guardian
The parent/guardian undertakes to assist Collab Teaching in identifying problem areas in which the student needs specific tutoring.
The parent agrees to provide the student with their own materials needed for each tutoring session (paper, pencil etc.)
Cancellation & Refund
The student/parent/guardian may cancel a session up to 24 hours in advance without being charged. Sessions not cancelled within the timeframe will be charged at the full tutoring session rate.
If Collab Teaching cancels a session for any reason at any time prior to the start of the session, Collab Teaching agrees to refund the student/parent/guardian any pre-payment for that session or to apply that payment to a future session.
Late arrival and change of schedule
Fees are calculated according to the scheduled time agreed upon by Collab Teaching and Student/Parent/Guardian. No adjustment to fees shall be made for any time lost because of late arrival by student/parent/guardian or by early termination by student/parent/guardian. If the student is dropped off later than the scheduled time, the tutoring session will still end at the regular scheduled time. Same fees will apply.
Collab Teaching agrees to wait 15 minutes from the time of the scheduled session for the student’s arrival before considering that session canceled without sufficient prior notice and therefore subject to the terms set forth in the cancellation and refund policy (see above).
Late Pickup
You will be charged $1.00 USD per minute if the child is picked up later than scheduled. Late fees will not be charged for the first five minutes of a late pick up. Please be cautious with time due to scheduling. We schedule students back-to-back.
Obligations of Collab Teaching
Collab Teaching undertakes to do all preparation prior to lessons and to structure lessons in such a way as to optimize time to the benefit of the student.
Collab Teaching shall keep all information confidential and will only share information with parties listed on the intake form.
Collab Tutoring shall at no time be required or obligated to execute homework or assignments.
Promises
Collab Teaching makes no promises or warranties with regards to a student’s performance as a result of any tutoring provided.
First Session
Collab Teaching will provide a placement assessment and an interest survey to the student, which will allow Collab Teaching to better plan for future sessions. Specific tutoring may not be provided the first session. Building rapport and a good relationship with the student is very important to Collab Tutoring.
Progress Monitoring
If the student/parent/guardian would like to have a conference about growth, please let Collab Teaching know. We will complete the meeting during one of the scheduled sessions. Unfortunately, due to scheduling, we cannot have a conference at arrival or departure time.
Toileting & Bathroom
Collab Teaching will not assist in the bathroom. If the student is not potty trained, or needs assistance in the bathroom, we do require you to stay on premises.
Virtual Sessions
The same policies apply to virtual sessions as in-person sessions. All virtual sessions will be conducted via Zoom. By signing you give your child permission to participate in Zoom Calls.
Virtual Session Agreement

Students/parents/guardians/Collab Teaching will be dressed appropriately.

Student will pay attention just like you would at school. No playing on the computer, no video games while in the tutoring session.

Student will not use the Zoom chat box unless Collab Teaching requests.

Student/parent/guardian will be aware of surroundings. Please choose a place that is peaceful and not distracting.

Stay in one place.
In Home Office - 18879 Diego Location

At the home office, no additional guests/children are allowed, other than parent/guardian and student who is being tutored.

There is a RING camera, which does record. Although, you cannot watch the session live from it.

The home office does have a 4lb Yorkie. She will be upstairs and away from the family.

The parent/guardian can sit in the waiting area or wait in their car.

For the months of October-December scheduling may be altered or cancelled based on availability.

This location follows the Pasco and Hillsborough County school calendar.
Minors
Where the student is a legal minor, the parent/guardian shall enter this tutoring contract on behalf of the student and shall accept and agree to all the terms and conditions contained herein on behalf of the student.
Whole Agreement
This agreement constitutes the entire understanding between the parties with regard to the subject matter thereof and the parties waive the right to rely on any alleged expressed or implied provision not contained herein. Any alteration of this agreement
must be in writing and signed by both parties.
Social Media Release
Social media is a great way to stay connected! Throughout the tutoring sessions, we will be posting pictures of students during various learning experiences on our website, Facebook, Twitter, Instagram and/or TikTok. Personal information will NEVER be shared.
